Privacy Policy

This Privacy Policy explains how Harvey Transportation Assistance Foundation (HTAF), a Tennessee 501(c)(3) public charity (EIN 41-5115030), collects, uses, discloses, and protects information in connection with transportation-assistance applications submitted through harveytransportationfoundation.com. It applies only to HTAF's charitable application portal — it does not describe or govern any separate commercial ride-hailing service.

1. Information We Collect

When you submit a transportation-assistance application, we may collect:

• Applicant contact information: name, email address, phone number, county, and city.
• Household and income information: applicant type (self, parent/guardian, caregiver, case worker, social worker), household size, and monthly household income, where voluntarily provided to support eligibility review.
• Transportation-need information: a description of the assistance requested, the program category (medical, employment, education, or community), and relevant dates or destinations.
• Uploaded supporting records: documents you choose to upload to support your application, such as appointment confirmations, TennCare or insurance documentation, disability or veteran verification, employment or enrollment records, or referral letters.
• Communications: information you provide when you contact HTAF by phone, email, or through the website.

We do not collect precise real-time location data, GPS tracking data, or payment card information through this application portal.

2. How We Use Information

Information submitted with an application is used to:

• Evaluate eligibility for HTAF's transportation-assistance programs;
• Verify the information provided, including by contacting the applicant or a named case worker or referral source;
• Coordinate transportation assistance for approved requests, including with service providers described below;
• Maintain records required for HTAF's grant reporting, donor accountability, and 501(c)(3) compliance obligations;
• Communicate with applicants about the status of an application; and
• Improve HTAF's programs and understand community transportation needs in the aggregate.

Submitting an application does not create an obligation for HTAF to provide assistance. Every application is reviewed individually against eligibility, documentation, and available funding.

3. Service Providers

HTAF may share limited application information with third parties who help operate its programs, such as hosting and email-delivery providers, and, where an application is approved, transportation providers or volunteer drivers who need applicant contact and trip information to complete the assistance requested. Service providers are permitted to use information only to perform services for HTAF and are not authorized to use it for their own independent marketing purposes.

HTAF does not sell applicant information, and does not share it with advertisers or data brokers.

4. Retention

HTAF retains application information for as long as reasonably necessary to evaluate the application, provide approved assistance, and satisfy legal, accounting, grant-reporting, and nonprofit recordkeeping requirements. Information no longer needed for these purposes is deleted or de-identified on a periodic basis.

5. Security

HTAF uses reasonable administrative, technical, and physical safeguards designed to protect application information against unauthorized access, alteration, or disclosure. No method of transmission or storage is completely secure, and HTAF cannot guarantee absolute security.

6. Applicant Choices

• You may decline to provide optional information (such as household income), though doing so may limit HTAF's ability to evaluate your eligibility.
• You may request access to, correction of, or deletion of the personal information you submitted, subject to HTAF's recordkeeping obligations, by contacting HTAF using the information below.
• You may withdraw an application at any time before a funding decision is made by contacting HTAF.

7. Children's Privacy

HTAF's application portal is intended for use by adults applying on their own behalf or as a parent, guardian, caregiver, or case worker applying on behalf of another person. HTAF does not knowingly collect personal information directly from children under 13 through this website. Information about a minor may be submitted only by that minor's parent, guardian, or authorized caregiver as part of an application for transportation assistance.

8. Health Information

Some applications describe a medical appointment or health-related transportation need. HTAF is not a healthcare provider, health plan, or health care clearinghouse, and does not represent that it is compliant with the Health Insurance Portability and Accountability Act (HIPAA). Where a referring healthcare organization is itself subject to HIPAA and shares information with HTAF in connection with a referral, that organization's own HIPAA obligations govern its handling of that information; HTAF will enter into a Business Associate Agreement with such an organization where one is legally required. HTAF uses any health-related information an applicant chooses to submit solely to evaluate and, if approved, coordinate the requested transportation assistance.

9. Changes to This Policy

HTAF may update this Privacy Policy from time to time to reflect changes in its programs or legal requirements. Updated versions will be posted on this page with a revised effective date.
